How information is used
Information is used to review your request and contact you about availability, arrangements and charges. Appointment requests are not confirmed bookings.
Keep sensitive information out
Do not submit document contents, Social Security numbers, identification copies, financial account details or medical information. The form does not accept document uploads.
Request changes or removal
Use the appointment form to request a correction or deletion, referencing your original request number. Notarial journal records, where required, are separate and subject to applicable retention requirements.
